How the Pathways HUB Works
CHWs are coaches and advocates. They help navigate social service systems, connect program participants to medical care, and ensure a long-term plan is in place to consistently keep your basic needs met.

Enroll
After you complete a referral form and qualify, a community health worker (CHW) from a partnering agency will contact you to meet at your home.
Meet & Assess
Next, you will answer a series of questions with your CHW. You’ll work with your CHW to prioritize needs and meet in-person once a month.
Plan
The CHW develops a care plan by opening “pathways” for each unmet need such as: health insurance, food security, housing, and transportation.

Queens Village Toledo partners with the Northwest Ohio Pathways HUB, providing a unique approach to address racial disparities in infant mortality. By creating safe spaces, Black women can find support, access health & advocacy resources, and alleviate chronic stress.

The Keystone Fund
Since 2021, the Keystone Fund has helped clients take the final step toward safe, stable housing. With your support, more families can cross that threshold. Your gift to the Keystone Fund helps families overcome financial barriers by providing up to $500 for critical expenses like deposits and move-in costs.

The Story Behind Our Logo
The HUB logo reflects both our connection to HCNO and the unique mission of the HUB. The colors and fonts are aligned with HCNO’s brand identity, reinforcing that the HUB is part of a larger commitment to building stronger, healthier communities.
At the same time, the HUB logo introduces a new icon that sets it apart while capturing its purpose. The circular symbol represents connection, collaboration, and change. The shapes within the circle resemble people gathered together, symbolizing the individuals, families, CHWs, and partners who are all part of the HUB model. Each figure points toward the center, representing unity and the collective effort to create healthier outcomes.
The movement within the icon also reflects the HUB’s role in guiding people along their pathways to better health. It shows that change is possible, and that through support and connection, lives can transform in new, positive directions.
