Community Health Assessments (CHAs) and Community Health Needs Assessments (CHNAs)

We provide full-service community health assessments tailored to your needs, including:
 

  • Primary Data Collection – Online surveys, mailed surveys, focus groups, and key informant interviews
     

  • Secondary Data Collection – Comprehensive, county-level data on health outcomes and social determinants of health
     

  • Sampling Options – Convenience and purposeful sampling (online) and representative sampling (mailings) to ensure accurate, actionable results
     

  • Qualitative Data Collection - Focus groups, key informant interviews, community perception surveys
     

  • Compliance Support – Meets Ohio Revised Code and IRS requirements and PHAB accreditation standards, while aligning with the State Health Assessment (SHA) and State Health Improvement Plan (SHIP).

We facilitate strategic, evidence-based planning to address identified health priorities, utilizing the Mobilizing for Action through Planning and Partnerships (MAPP) framework:

 

  • MAPP Framework in Action – Structured planning meetings with community stakeholders to guide the process from visioning to priority setting.
     

  • Goal & Strategy Development – Clear action steps with measurable objectives tailored to community needs.
     

  • Full Technical Writing & Facilitation – From meeting facilitation to delivering the final CHIP report.
