Through collaboration with local partners and community health workers (CHWs), the Northwest Ohio Pathways HUB supports enhancing health outcomes, promoting healthy births, and enabling connections to health & social services for Northwest Ohio residents impacted by social risk factors.
The Pathways HUB system partners with CHWs and other care coordinators employed by clinical and community sites. Through developed care plans, program participants are provided tools to overcome barriers to lead healthier lives. Pathways HUB staff coordinates with various care coordination agencies throughout Northwest Ohio and tracks program results.
