Health Assessments

 

To view individual county health assessments, please click the map below. For each county that has participated you can view the health assessment report. You will find health-related data for adults (19 years of age and older) and youth (ages 12 through 18). The findings are based on self-administered surveys using a structured questionnaire. The questions were modeled after the survey instruments used by the Center for Disease Control and Prevention for their national and state Behavioral Risk Factor Surveillance System (BRFSS) and Youth Risk Behavior Surveillance Survey (YRBSS), therefore a county can compare themselves with state and national data. All participating counties areas also included in the NW Ohio Regional Health Assessment Report. This is updated twice a year. Please click on the link below to view. Data from the individual reports have been compiled from the following counties: Erie, Fulton, Henry, Ottawa, Sandusky, Seneca, Wyandot, Williams, Lucas, and Huron. Wood and Auglaize counties will be starting the process in spring 2008. This process will hopefully be repeated in each county every 3-5 years. The Health Communities Foundation of the Hospital Council of Northwest Ohio collected the data, guided the health assessment process and integrated sources of primary and secondary data into individual county reports.
